Wisconsin Immunization Registry (WIR) School Renewal for the 2024-2025
Reminder: Each school will have only one (1) account, which MUST be registered to the principal (or head of school), responsible for that school.
Established Principals: IF you are the principal for the same school as last year, change your account password on or after April 1, 2024.
- Login to WIR using the org code, username & password specific to you/your school
- Select Change My Password under Manage My Account
- Update your password
New Principal for a school: You MUST have your own account affiliated with the school
- Login to WIR using the prior account holder credentials for the school
- Select r/r tab at the top of the page
- Select the School Renewal/Enrollment link
- The School Renewal Form page loads your school information
- Update the appropriate fields with your name, email, and phone number
- Click Save and then Approval Form(s)
- Print the forms, sign them, and email them to DHSWIRHelp@dhs.wisconsin.gov. DO NOT alter generated forms
IMPORTANT : If you are renewing multiple accounts, you MUST login to each school separately to complete the process. DO NOT erase and refill the form in lieu of logging in under the next account.
