School Renewals for the 2026/2027 School Year

All schools using the Wisconsin Immunization Registry (WIR) must renew their account before the 2026/27 school year. The WIR renewal window is now open, and schools may begin the process at their convenience.

Only the account holder for each school should complete the renewal process. The account holder must be the principal or head of the school (for example, dean). Individual user accounts cannot be issued to other staff members, such as nurses or support staff.

To ensure successful processing, please follow these requirements:

Renewals must be submitted by each individual school, not the district.
Use only the official school renewal form(s) generated by WIR.
The form(s) must be signed by the principal only.
Do not handwrite or cross out information on the form.
Submit one renewal per school by email, fax, or mail. Choose only one method.

Before starting, please have the following information ready:

WIR login information - Organization code, Username, and Password

If you do not have a WIR account or have forgotten your login information, contact the WIR Help Desk at DHSWIRHelp@dhs.wisconsin.gov or (608) 266-9691 for assistance.

School and contact information - This information will automatically populate on the renewal form. Update any details that have changed.
